Community Alternative Program/Disabled Adults (CAP/DA)
The CAP/DA program is a Medicaid program that assists qualified individuals to receive supportive care and services in their own home, as an alternative to nursing home placement.
Some of the support services available include:
Case management provided by registered nurses and social workers
In-home aide service
Adult day health
Durable medical equipment
Medical supplies
Telephone/lifeline alert
Home modifications
CAP/Choice:
This CAP option is consumer driven care. Once approved for CAP/Choice, this program allows older and disabled individuals to have the opportunity to maintain increased control over their care, support and services arranged to meet their needs.
Who is eligible for CAP/DA or Choice?
Polk County residents
Those who are eligible for Medicaid
Those at risk for nursing home placement
Individuals who can be approved for skilled or intermediate care by a physician
Individuals who can be cared for safely in their home at less cost than facility placement
