St. Luke's Hospital Diabetes Self-Management Education Program

Accredited by the American Association of Diabetes Educators

The Diabetes Self-Management Education Program at St. Luke's Hospital focuses on problem-solving and goal-setting to meet the needs of each individual patient. The Core Curriculum consists of two classes that are interactive and discussion-based, followed by 1:1 meetings with the certified diabetes educator to set goals and address individual challenges. Participants may also choose to meet with the diabetes educator 1:1 only.

In the program, participants learn how to manage all aspects of diabetes, from diet and exercise, monitoring and medications, to psychological and psychosocial stress. Referral from the healthcare provider who manages the person's diabetes is required, and the health care provider receives reports about the problem-solving and goal-setting discussions with the patient.

Program Staff

Melissa Melum, RN, CDE is a registered nurse diabetes care and education specialist. She has worked with patients across the spectrum of diabetes, from newly diagnosed to patients on insulin pumps. Since 2013, she has worked at St. Luke's Hospital on interdepartmental support of inpatient diabetes management, and she has also worked at Polk Wellness Center where she developed their outpatient diabetes self-management education program.
