RN Case Manager - Full-time/Days: Case Management - Case Management
Position: RN Case Manager
Location: Columbus, NC

St. Luke’s Hospital has an opportunity for an experienced RN Case Manager for our Case Management Department! In this role, you will be part of a progressive team dedicated to advancing the goals of the hospital, meeting the needs of our rural community, and providing outstanding care to our patients. Essential Functions:
• Coordinates/ consults patient care/ admissions with all disciplines to ensure interdisciplinary approach to the patient’s needs and care.
• Performs insurance reviews, initial reviews and updates as required.
• Have knowledge of medications and their correct administration based on age of the patient and their clinical condition.
• Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, religious/spiritual needs of patients and their families.
• Functions as liaison between administration, patients, physicians and health providers.
• Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age-appropriate care to the patient population served. Able to interpret information about the patient’s status to identify each patient’s age specific needs and provide necessary care.
• Assists with Medicare authorization and re-certification on Swing Bed patients.
• Assists in the review and evaluation of data, determining the presence or absence of opportunities for improvement of care.
• Demonstrates an ability to be flexible, organized and function under stressful situations.
• Is knowledgeable of criteria for Medicare, Medicaid, HMO and private insurance companies.
• Is knowledgeable of the patient’s financial status, diagnosis and discharge needs.
• Maintains a good working relationship both within the department and with other departments.
• Actively participates in performance improvement and continuous performance improvement (CQI) activities.

Required Knowledge, Skills, and Abilities:
• Education: BSN preferred in Nursing or related field or AND with 3 years Case Management experience and expectation of BSN completion within 5 years.
• Work Experience: Two years RN experience required with clinical experience in nursing care of the medically complex patient or community care desirable. Prefer experience in rural critical access hospital setting.
• Licenses & Certifications: Maintain state RN licensure in state where employed. ACM or CCM highly desirable but not required initially, must be obtained within 3 years.
• Maintain CPR certification.
• Knowledge and skill in chronic disease management.
• Excellent verbal and written communication (including documentation)
• Excellent customer service/ relationship management with patients, families and all other care providers.
• Professional demeanor
• Flexible in trying new things while developing departmental model or a new care program.
• Able to work as a member of a team
• Strong ability to organize and prioritize.
• Basic computer skills needed to navigate EMR. WORD and Excel experience desirable.
• Appropriate communication skills. Additional languages helpful, but not required.
