Pre-Employment Appointment

Everything about our New Teammate process is designed to help make your onboarding easier and more manageable.

You will meet with Employee Health and then with Human Resources. On average, this process takes two to five hours.

Onboarding Documents


Please ensure that you bring all of the following:

A1a Employee Contact Information
A5 Employee's Withholding Allowance Certificate W-4
A6a Employee's Withholding - NC-4 - give NC-4 & NC-4EZ to employee to decide
A6b Employee's Withholding - NC-4EZ - give NC-4 & NC-4EZ to employee to decide
Attendance Policy
Applicant Disclosure and Consent For Background Investigation Form
Dress Code & Professional Appearance
Veteran Disclosure and Background Discloser
Human Resources
1. Valid, unexpired ID(s) from this list of Acceptable Documents to prove you are authorized to work in the United States.

2. A voided check or form from your bank stating your bank Routing & Account Number for Direct Deposit.

3. Copies of your Clinical Certification(s) (if applicable) i.e.: ACLS, BLS, PALS, NCI, TNCC, etc.

Note: We will take your picture for your employee badge during your appointment.
